Program Guidelines

After initial sign-up, there will be no paperwork to complete. Our office will conveniently apply your discounts at each visit. All annual enrollment fees must be paid in advance and the plan is active for one year from the effective date. Payment for your discounted services is due at the time of treatment. Refunds of annual enrollment fees will not be issued if a plan member decides not to take advantage of plan benefits or following 30 days from enrollment. You will not receive a membership card. The effective date and enrollment form will be on file in our office.

This program is a discount plan, it is not dental insurance. It cannot be used:

• In conjunction with another dental plan or dental insurance
• At any other dental office. This plan is only honored at Duxbury Dental, LLC.
• For treatment, which, in the sole opinion of our doctor is outside the scope of her capability.
• At dental specialists or other specialists to which plan members are referred.
• For hospitalization or hospital charges of any kind.
• For costs of dental care which are covered under medical insurance.
• For services relating to injuries covered under worker’s compensation.
• In conjunction with third party financing companies.
